Everyone deserves great healthcare. Older adults have specific healthcare needs. Many don’t receive it. When they do, appointments are brief, in-network doctors can be far away and they don’t get personalized treatment plans. We’re changing all of that. 01 Personal Nothing is more personal than your health. Our care is personal, spending significantly more time with our patients and knowing them as individuals. 02 Equitable You should have access to the same great care no matter where you live. We invest in our communities by building Oak Street Health centers where healthcare services are needed most. 03 Accountable Many adults on Medicare have chronic and complex conditions. We hold ourselves accountable for our patients’ health by taking on the risk and cost of their care. Value Over Volume We’re all in on helping patients live healthier. Most primary care practices follow fee-for-service models, which often prioritize volume of patient visits over quality of care. Oak Street Health largely operates under a value-based managed care model, meaning our approach aligns the incentives of our patients, our providers and our payors by simultaneously improving health outcomes and care quality, lowering medical costs and improving the patient experience. Doctor Patient Consult What it means to be Oaky. All Oakies are mission-driven individuals who strive to go above and beyond to support their coworkers and our patients. We work as a team and across disciplines to rebuild healthcare as it should be. Creating patient experience Creating an unmatched patient experience We’re driven to provide the best healthcare experience our patients have ever had. We go to extraordinary lengths to ensure their well-being. Driving clinical excellence Driving clinical excellence We invest in innovation, access and new services to push primary care to new frontiers. This helps us provide outstanding care to all our patients. Taking ownership Taking ownership and delivering results Our mission is too important to have small goals. We learn from failure and push ourselves, and each other, to continually improve, collaborate and innovate. Being scrappy Being scrappy We move fast and operate in a highly ambiguous environment. We make hard decisions, with imperfect information, knowing we’ll have to learn and adjust on the fly. Radiating positive energy Radiating positive energy Positive energy inspires the best in ourselves and our co-workers. People with positive energy are fun to work with and find joy working on our most ambitious goals. Assuming good intentions Assuming good intentions We strive to embody trust, empathy and integrity. We communicate with candor, provide direct feedback and expect the same of our colleagues. Providing access where it is needed most. In 2013, we opened our first center in Chicago’s Edgewater neighborhood. Oak Street Health continues to expand to new cities and states.
